Grant of Rights

I, the undersigned Model, hereby grant to Michael Merrill, DBA MMco Design (“Art Director”), and its legal representatives, licensees, and assigns, the irrevocable and unrestricted right to use and publish photographs, video, or other media of me, or in which I may be included, for editorial, advertising, trade, promotional, or any other lawful purpose, in any manner or medium.

Compensation & Expenses

As full consideration for this release, I agree to the following compensation:

• $300 model fee

• $40 gas stipend

• Lunch provided on the day of the shoot

• Hotel accommodations paid by MMco Design

I understand that no further compensation will be due beyond what is stated above.

Consent & Waiver

• I understand these materials may be used in print, online, or other digital formats.

• I waive any right to inspect or approve the finished images, advertising copy, or other matter used in connection therewith.

• I release the Art Director and all persons acting under their permission from any liability for blurring, distortion, alteration, or optical illusion, whether intentional or otherwise.

• I affirm that I am not relying on any other representations outside of this agreement.

Acknowledgment

I am of legal age (18+) and have the right to contract in my own name. If under 18, my parent/guardian has signed below.
